OBJECTIVES/GOALS: A particularly debilitating consequence of stroke is alexia, an acquired impairment in reading. Cognitive models aim to characterize how information is processed based on behavioral data. If we can concurrently characterize how neural networks process that information, we can enhance the models to reflect the neuronal interactions that drive them. METHODS/STUDY POPULATION: There will be 10 unimpaired adult readers. Two functional localizer tasks, deigned to consistently activate robust language areas, identify the regions of interest that process the cognitive reading functions (orthography, phonology, semantics). Another task, designed for this experiment, analyses the reading-related functional-connectivity between these areas by presenting words classified along the attributes of frequency, concreteness, and regularity, which utilize specific cognitive routes, and a visual control. Connectivity is analyzed during word reading overall vs. a control condition to determine overall reading-related connectivity, and while reading words that have high vs. low attribute values, to determine if cognitive processing routes bias the neural reading network connectivity. RESULTS/ANTICIPATED RESULTS: The localizer analysis is expected to result in the activation of canonical reading areas. The degree of functional connectivity observed between these regions is expected to depend on the degree to which each cognitive route is utilized to read a given word. After orthographic, phonologic, and semantic areas have been identified, the connectivity analysis should show that there is high correlation between all three types of areas during reading compared to the control condition. Then the frequency, regularity, and concreteness of the words being read should alter the reliance on the pathways between these area types. This would support the hypothesized pattern of connectivity as predicted by the cognitive reading routes. Otherwise, it will show how the neural reading network differs from the cognitive model. DISCUSSION/SIGNIFICANCE OF IMPACT: The results will determine the relationship between the cognitive reading model and the neural reading network. Cognitive models show what processes occur in the brain, but neural networks show how these processes occur. By relating these components, we obtain a more complete view of reading in the brain, which can inform future alexia treatments.

The most important of Lenin's writings was, arguably, Imperialism: the Highest Stage of Capitalism. That work shifted the focus from workers’ struggles within one country to the dynamics of capitalism as a global system. The Leninist project thereby inextricably linked the causes of economic justice and national liberation, a fateful step in light of the transformation of the world wrought by decolonization. As capitalism stumbles through yet another global crisis today, what parts of Lenin's fevered vision remain relevant 100 years later?

Summary
Introduction
John Friedman
The star has to go on radiating and radiating and contracting and contracting until, I suppose, it gets down to a few km. radius, when gravity becomes strong enough to hold in the radiation, and the star can at last find peace. Dr. Chandrasekhar had got this result before, but he has rubbed it in in his latest paper; and, when discussing it with him, I felt driven to the conclusion that this was almost a reductio ad absurdum of the relativistic degeneracy formula.
(A. S. Eddington [1])The emphasis of this chapter is on four parts of relativistic astrophysics in which general relativity plays a fundamental role. After briefly reviewing the early history of the subject, we discuss
The structure and stability of relativistic stars
Observational evidence for black holes
Gamma-ray bursts
Gravitational lensing
General relativistic astrophysics encompasses a broader arena, and separate chapters or parts of chapters in this volume are devoted to cosmology, gravitational waves, the inspiral and merger of compact binaries, and black-hole stability.
Relativistic astrophysics began in 1916 on the Russian front, where Karl Schwarzschild wrote two papers, one reporting the solution to the Einstein equation for an incompressible spherical star, the other presenting the celebrated vacuum Schwarzschild spacetime. Schwarzschild was dead within the year, and for the next 47 years his solutions had a twilight existence. In no known stars did general relativity play a significant role, and only a handful of papers in astronomy or astrophysics mentioned the work.
Although sparsely distributed, the exceptions to this neglect were remarkable. In 1931, shortly before Chadwick's discovery of the neutron and shortly after the first paper by Chandrasekhar [2] (following approximate computations by Anderson [3] and Stoner [4] on an upper mass limit of white dwarfs, Landau [5] submitted a paper that independently argued that there was an upper limit on the mass of a collection of degenerate fermions and speculated on the existence of stars with cores of nuclear density.

This paper deals with the question whether the assumption that for every inaccessible cardinal κ there is a well-order of H(κ+) definable over the structure $\langle {\rm{H}}({\kappa ^ + }), \in \rangle$ by a formula without parameters is consistent with the existence of (large) large cardinals and failures of the GCH. We work under the assumption that the SCH holds at every singular fixed point of the ℶ-function and construct a class forcing that adds such a well-order at every inaccessible cardinal and preserves ZFC, all cofinalities, the continuum function, and all supercompact cardinals. Even in the absence of a proper class of inaccessible cardinals, this forcing produces a model of “V = HOD” and can therefore be used to force this axiom while preserving large cardinals and failures of the GCH. As another application, we show that we can start with a model containing an ω-superstrong cardinal κ and use this forcing to build a model in which κ is still ω-superstrong, the GCH fails at κ and there is a well-order of H(κ+) that is definable over H(κ+) without parameters. Finally, we can apply the forcing to answer a question about the definable failure of the GCH at a measurable cardinal.

Background:
Guillain-BarrÈ syndrome (GBS) classically presents with a subacutely evolving areflexic paralysis, with typical laboratory findings of elevated cerebrospinal fluid protein and abnormal nerve conduction studies. There is now an increasing recognition of GBS variants that differ in clinical presentation, prognosis, electrophysiology and presumed pathogenesis. Fulminant cases of GBS have been reported in which a rapid deterioration evolves to a clinical state resembling ìbrain deathî.
Methods:A retrospective analysis of two such cases of fulminant neuropathy are described, that includes the clinical course, electrophysiology and neuropathology where available.
Results:We describe two patients that presented with a rapid course of neurological deterioration, lapsing into what resembled a ìclinically brain-deadî state that was subsequently ascribed to a fulminant polyneuropathy. Investigations (electrophysiological, pathological) and the clinical course suggested an axonal neuropathy.
Conclusion:A fulminant neuropathy can result in a clinical state resembling ìbrain deathî through diffuse de-efferentation. Although generally attributed to aggressive demyelination with secondary axonal degeneration, a primary axonopathy can also lead to a similar clinical presentation.

Introduction: Annual ambulance diversion hours in Boston increased more than six-fold from 1997 to 2006. Although interventions and best practices were implemented, there was no reduction in the number of diversion hours.
Objectives: A consortium of Boston teaching hospitals instituted a two-week moratorium on citywide diversion from 02 October 2006 to 15 October 2006. The hypothesis was that there would be no significant difference in measures of hospital and emergency medical services (EMS) efficiency compared with the two weeks immediately prior.
Methods: A total of nine hospitals and the municipal emergency medical services in Boston submitted data for analysis. The following mean daily hospital measures were studied: (1) emergency department volume; (2) number of emergency department admissions; (3) length of stay (LOS) for all patients; and (4) number of elopements. Mean EMS at-hospital time by destination and the percent of all Boston EMS transports to each hospital destination were calculated. The median differences (MD) were calculated as “before” minus “during” the study period and were compared with paired, Wilcoxon, non-parametric tests. Additional mean EMS measures for all destinations included: (1) to hospital time; (2) number of responses with transport initiated per day; (3) incident entry to arrival; and (4) at-hospital time.
Results: The LOS for admitted patients (MD = 0.30 hours; IQR 0.10,1.30; p = 0.03) and number of daily admissions (MD = -1.50 patients; IQR -1.50, -0.10; p = 0.04) were significantly different statistically. The results for LOS for all patients, LOS for discharged patients, ED volume, EMS time at hospital by destination, number of elopements, and percent of Boston EMS transports to each hospital revealed no statistically significant differences. The difference between the study and control periods for mean EMS to hospital time, at-hospital time, and incident entry to arrival was a maximum of 0.6 minutes. The vast majority of EMS respondents to an online survey believed that the “no diversion” policy should be made routine practice.
Conclusions: The LOS for admitted patients decreased by 18 minutes, and the number of admissions increased by 1.5 patients per day during the study period. The “no diversion” policy resulted in minimal changes in EMS efficiency and operations. Diversion was temporarily eliminated in a major city without significant detrimental changes in ED, hospital, or EMS efficiency.

Objective.
To determine the epidemiological characteristics of postoperative invasive Staphylococcus aureus infection following 4 types of major surgical procedures.
Design.Retrospective cohort study.
Setting.Eleven hospitals (9 community hospitals and 2 tertiary care hospitals) in North Carolina and Virginia.
Patients.Adults undergoing orthopedic, neurosurgical, cardiothoracic, and plastic surgical procedures.
Methods. We used previously validated, prospectively collected surgical surveillance data for surgical site infection and microbiological data for bloodstream infection. The study period was 2003 through 2006. We defined invasive S. aureus infection as either nonsuperficial incisional surgical site infection or bloodstream infection. Nonparametric bootstrapping was used to generate 95% confidence intervals (CIs). P values were generated using the Pearson x2 test, Student t test, or Wilcoxon rank-sum test, as appropriate.
Results. In total, 81,267 patients underwent 96,455 procedures during the study period. The overall incidence of invasive S. aureus infection was 0.47 infections per 100 procedures (95% CI, 0.43–0.52); 227 (51%) of 446 infections were due to methicillin-resistant S. aureus. Invasive S. aureus infection was more common after cardiothoracic procedures (incidence, 0.79 infections per 100 procedures [95% CI, 0.62–0.97]) than after orthopedic procedures (0.37 infections per 100 procedures [95% CI, 0.32–0.42]), neurosurgical procedures (0.62 infections per 100 procedures [95% CI, 0.53–0.72]), or plastic surgical procedures (0.32 infections per 100 procedures [95% CI, 0.17¬0.47]) (P < .001). Similarly, S. aureus bloodstream infection was most common after cardiothoracic procedures (incidence, 0.57 infections per 100 procedures [95% CI, 0.43–0.72]; P < .001, compared with other procedure types), comprising almost three-quarters of the invasive S. aureus infections after these procedures. The highest rate of surgical site infection was observed after neurosurgical procedures (incidence, 0.50 infections per 100 procedures [95% CI, 0.42–0.59]; P < .001, compared with other procedure types), comprising 80% of invasive S. aureus infections after these procedures.
Conclusion. The frequency and type of postoperative invasive S. aureus infection varied significantly across procedure types. The highest risk procedures, such as cardiothoracic procedures, should be targeted for ongoing preventative interventions.
